CCNB works with the Northern Sydney Local Health District Aboriginal Health Unit to support First Australians to access timely services.
This time limited project is a community development project that engages a respected, experienced community development worker, passionate about the health and wellbeing of First Australians.
The model of service is focused on three core areas:
Our Care Coordination service is designed to work with people, their families, carers and decision makers to understand the local health and social care system. Your Care Coordinator works collaboratively with you to personalise your supports and match you with the right staff and services to meet your needs and ensure you get the most out of life.
Community Capacity Building
We continue to build community capacity through sector training, information exchange, events and contributing to policy development and practice. This important work ensures our local community is well informed and able to respond to the needs of our First Australians.
We work with others on projects and initiatives to improve the health and wellbeing of local First Australians.
