Northern Beaches Community Care Packages
This project is funded by the Australian Department of Health and Ageing. The project provides packages of care to older people in Manly, Warringah and Pittwater local government areas. The coordinators provide individualised case management and monitoring to each care recipient and direct care services are brokered through a private agency with one primary care worker matched to each care recipient.
CACPs are packages of care, which can assist older people who wish to remain living independently in the community. The assistance is provided in a flexible and coordinated way in order to best meet individual needs.
Older people who need a coordinated package of services in order to live independently in the community.
· A co-ordinator will visit the person at home to discuss their individual needs through a comprehensive assessment process. · In consultation with the person and if appropriate, their family or carer, the coordinator will develop a care plan. · The care plan may include services such as:
- Personal care; - Meal preparation - Home and garden maintenance; - Transport to appointments and recreational activities; - Respite for the carer.
· Having developed the care plan the coordinator will discuss an appropriate contribution towards the cost of the service. · The coordinator will provide a Service Agreement, including rights and responsibilities of the client, complaints procedures and information regarding privacy and confidentiality policies. · The coordinator will arrange the services agreed to in the care plan and will monitor and adjust the services according to the changing needs of the client. · Experienced direct care workers will be used to meet client needs.
A weekly fee will be discussed dependent on ability to pay. The Aged Care Act determines that a person in receipt of the Aged Pension will pay no more than 17½% of the pension towards the cost of the Community Aged Care Package. No-one will be excluded due to inability to pay.
Anyone can make a referral but in the first instance eligibility for a Community Aged Care Package needs to be determined by an Aged Care Assessment Team. This assessment is free.
