Extended Aged Care at Home (EACH)
Extended Aged Care at Home This Project is funded by the Australian Department of Health and Ageing and is a package of care which can assist an older person with high complex care needs who wishes to remain living independently in the community. Case Managers provide an individualised service and direct care brokered through private agencies.
