Northern Beaches Transpac

Transpac is jointly funded by the Australian Government and NSW Health through Northern Sydney Central Coast Area Health Service as part of the national Transitional Care Program. Transpac operates in Northern Sydney to assist eligible clients to discharge from public and private hospitals, when their other option may be residential care. Transpac provides a case managed package of support for an average of 9, and up to 12 weeks, which includes in-home support services and allied health therapies to assist clients to improve their level of functioning in order to be able to remain safely in their own homes. 


HOW DOES IT WORK?


A Case Manager will usually visit you in hospital and discuss your individual needs with you, the hospital team and your family, if you  wish.  A program of home support services and therapy will be agreed.  Your Case Manager will keep in regular contact to monitor your progress and needs.  Services will be gradually reduced as your condition improves.  Your General Practioner will look after your medical care.


WHAT DOES IT COST?


There will be a client contribution of equivalent to 17.5% of the basic daily rate of the single aged pension.  The amount charged for Transpac will form part of the agreement between yourself and Community Care (Northern Beaches) Inc.  No person will be denied access to Transpac because of lack of capacity to pay.


WHO CAN MAKE A REFERRAL?


Hospital staff who refer an inpatient to Transpac include social workers, discharge planners, discharge liaison service, continuum of care coordinators.


WHAT HAPPENS AT THE END OF THE TRANSPAC?


When you are nearing the end of the agreed time on Transpac, your Case Manager wil be talking with you, and your family if you wish, about the types of care you may need after the program or in the future, and will assist you in organising ongoing care.
