Northern Beaches Dementia Monitoring Project

This project is funded jointly by the Australian and New South Wales Governments under the Home and Community Care Program (HACC).  The funding is administered by the NSW Department of Ageing Disability and Home Care (DADHC).    The project monitors the health and well-being of people with dementia living alone in the community, with or without a primary carer. The project operates in a flexible way to contribute towards supporting people with dementia to maintain skills of daily living; maintain independence in meal preparation and eating; reduce social isolation and provide companionship.


WHAT IS THE MONITORING SERVICE?


The Monitoring Service provides a regular contact for people with memory loss who live alone in the community. The monitoring is provided in a flexible way in order to meet people’s individual needs.


WHO IS ELIGIBLE?


People with memory loss who live alone in the community and require regular monitoring of their well-being in order to maintain their independence.


HOW DOES IT WORK?


· The coordinator will take a referral and ascertain eligibility for the service
· The coordinator will visit the person at home to undertake an assessment. They will discuss what type of assistance would best meet the needs of the client. A monitoring plan will be agreed to which could take the form of:

- Access to social activities
- Companionship
- Assistance with shopping or household tasks
- Encouragement with meal preparation and eating
- Provision of a flexible drop in service

· The coordinator will provide a Service Agreement, including rights and responsibilities for the client, complaints procedures and information regarding privacy and confidentiality policies. If it is more appropriate this information will be provided to the carer.
· The coordinator will arrange the monitoring service and will review and adjust it as required. Experienced direct care workers with direct links to the coordinator, will be used to provide the service.
· The coordinator will refer to other services which may be required.


WHAT IS THE COST?


A contribution towards the cost of the direct service will be negotiated on an individual basis, dependent on the ability to pay. No-one will be excluded due to inability to pay.


WHO CAN MAKE A REFERRAL?


Anyone can make a referral to the service.
