Community Options - Northern Beaches

This project is funded jointly by the Australian and New South Wales Governments under the Home and Community Care Program (HACC). The funding is administered by the NSW Department of Ageing Disability and Home Care (DADHC). 

The role of this project is to provide case management to people with complex care needs, within the HACC target group.  A small allocation of flexible funding is available to subsidise the cost of services not available through HACC funded community services. 

The project includes advocacy on behalf of the client to organisations within, and beyond, the community sector. The project operates on an episodic basis, providing case management to people for as long as they need it, and allowing people to re-access the project in the future as their needs change.


WHO IS ELIGIBLE FOR COMMUNITY OPTIONS?


People with complex care needs who:

· Are frail aged
· Have moderate to severe disabilities
· Have dementia
· Are Carers of these people


HOW DOES IT WORK?


· A coordinator will take a referral and ascertain eligibility for the service.
· A co-ordinator will visit the person at home to discuss their individual needs through a comprehensive assessment process.
· In consultation with the person and if appropriate, their family and carers, the coordinator will develop a care plan.
· The care plan may include services such as:

- home nursing;
- housekeeping;
- meal preparation
- home and garden maintenance;
- transport to appointments and recreational activities
- respite for their carer

(These services may be provided by other government agencies, or by private providers).

· The coordinator will provide a Service Agreement, including rights and responsibilities of the client, complaints procedures and information regarding privacy and confidentiality policies.
· The coordinator will arrange the services agreed to in the care plan and will monitor and adjust the services according to the changing needs of the client.
· Experienced direct care workers will be used to provide the services to the client
· The coordinator is the contact for the client and carer and will liaise with other service providers regarding changes to the assistance required.


WHAT IS THE COST?


· the coordinator will discuss an appropriate contribution towards the cost of the service.
· When services are purchased through private agencies, the Community Options project may be able to assist with costs.
· Other government funded agencies charge their usual fees.
· There is no charge for assessment and ongoing case management.


WHO CAN MAKE A REFERRAL?


Anyone can make a referral to the service.
