CCNB History
1990

  • The Community Options Project was established under the auspice of Manly Warringah Community Information & Service Centre Inc. (now known as the Northern Beaches Community Services Ltd.).  The project was called the Manly Warringah Live at Home Program and was funded for 80 people. 
  • The office was located at the Nelson Heather Centre, Jacksons Road, Warriewood.

1992

  • The service expanded to include the HACC funded Dementia Brokerage Project for 18 clients.
  • The office relocated from Warriewood to 1/6 Waratah Street, Mona Vale.

1994

  • A new incorporated organisation was formed called Community Care (Northern Beaches) Inc. (CCNB).  The auspice of the Community Options and Dementia projects was transferred to this new organisation.
  • CCNB committed to participation in the Manly Warringah Pittwater Demonstration Project under the guidance of the NSW Office on Ageing (which was later absorbed into the new NSW Ageing and Disability Department (ADD).

1995

  • The Dementia Brokerage received funding to increase to 22 clients.
  • The Management Committee, staff and stakeholders under the guidance of Janet Green and Associates developed a 3 year strategic plan.
  • Introduction of a networked computer system to CCNB and linkage with participating services in Manly Warringah Pittwater (MWP) Demonstration Project through modems.

1996

  • A new Medications Policy was developed in consultation with Sydney Home Nursing Service (Manly Branch) and the Public Interest Advocacy Centre through Blake Dawson Waldron, Solicitors.
  • The organisation was internally restructured reducing case loads for coordinators and creating a new part time coordinator position and increasing management hours.

1997

  • Request from Warringah Council to auspice In-Home Podiatry Service for housebound and financially disadvantaged people in Warringah and Pittwater local government areas.
  • All coordination staff participated in Case Management training through Charles Sturt University, Bathurst campus.

1998

  • Phase two of restructuring created a manager’s position without case load.
  • Commenced In-Home Podiatry Project.
  • Changes to CCNB Constitution to reflect broader geographic boundaries.
  • Established Respite Brokerage Project for Lower North Shore and Manly Warringah Pittwater.
  • Introduced Care Manager software to manage client data for all projects. 
  • Received funding from Department of Health and Aged Care for establishment of 30 Community Aged Care Packages in Manly Warringah Pittwater.

1999

  • Developed new three-year Strategic Plan.
  • Reviewed staff remuneration and introduced revised structure.
  • Submitted successful proposal for extra Community Aged Care Packages – 15 new packages were commenced from December 1999.
  • Community Options, Dementia Brokerage and Respite Brokerage Projects monitored by the Ageing and Disability Department in June 1999.
  • Submitted proposal to Commonwealth National Respite for Carers Program for new respite project for carers of people with challenging behaviours.
  • Expanded office premises to provide meeting/lunch room space, storage space, and separate area for photocopier.

2000

  • Put proposal to the NSW Ageing and Disability Department for the amalgamation of the Community Options Project and the Dementia Brokerage Project – approval received for commencement on July 1, 2000.
  • National Respite for Carers Program project commenced from July to support carers of people with dementia with challenging behaviours who are unable to access usual respite services. 
  • Received ABN number and confirmation of charitable status from Australian Tax Office in preparation for introduction of Goods & Services Tax.
    Introduced salary packaging arrangements for all permanent staff under Fringe Benefits Tax legislation.
  • CCNB HACC funded projects were independently validated against the HACC Standards in September 2000, scoring a  “High” overall performance against the standards, and using the AIHW consumer survey as part of the measure.  
  • Participated with Northern Sydney Carer Respite Centre in project to maintain an extra residential respite bed in a Mona Vale Nursing Home.  Trial was cost neutral and has been extended due to success. 

2001

  • Successful submission to Department of Health and Aged Care resulted in commencement of 10 new Community Aged Care Packages from February.  Total CACPs for CCNB now 55.
  • Expanded accommodation to provide separate administration office, meeting room with storage, and 3 offices for coordination staff.
  • Successfully tendered to Department of Ageing and Disability for new Dementia Monitoring Project to commence in third quarter of 2001.
  • Successfully tendered to Department of Ageing and Disability for expansion of the Community Options Project from 102 to 117 people.
  • Reorganisation of staffing of different projects approved by Management for introduction in third quarter of 2001.

2002

  • Successful submission to Department of Health and Ageing for 5 new Community Aged Care Packages in Manly, three of which are for Veterans.  Establishment funding received from Department of Veterans Affairs to support these packages.
  • Received funding from Department of Ageing, Disability and Home Care to increase Community Options Project from 117 people to 125 people in January 2002.
  • Received increase in the amount of Client Subsidy for clients in the Community Options Project.  Average for 125 clients is currently $52 per client per week.
  • Developed a new 3-5 year Strategic Plan and a new Mission Statement for the organisation.
    Successful submission to the Department of Ageing, Disability and Home Care for the establishment of a Dementia Advisory Service in Manly, Warringah and Pittwater.

2003

  • Successful submission to Department of Health and Ageing for 10 Community Aged Care Packages.
  • Successful submission to Department of Health and Ageing for increase in funding for Dementia Respite to enable increase of carers to 12.
  • Ageing, Disability and Home Care for the establishment of a Dementia Planning and Service Development Project.
  • Produced Demographic Profile of People who are Ageing, people with disabilities and their carers in Manly Warringah Pittwater.
  • Introduction of ComPacks Pilot Project following trial undertaken with CCNB Community Options and Northern Sydney Health. 

2004

  • ComPacks Project established with three year funding to assist in discharge of eligible people from major public hospitals in metropolitan areas of NSW as well as some rural locations.
    Commenced providing Carer Packages funded through the Northern Sydney Commonwealth Carer Respite Centre.
  • Dementia Planning & Service Development Project produced comprehensive map of dementia services in Northern Sydney with extensive  recommendations for service improvement across the region.
  • Received funding from DADHC to increase the In-Home Podiatry Project from 95 clients to 200 clients and to include coverage of the Manly local government area.
  • Received donation of $6000 from Clubs NSW to assist in providing client equipment.
  • Implemented restructure of organisation including teams for Complex Care and Coordinated Care.
  • Moved to new accessible premises with improved amenities for staff and clients.
    Participated in Innovative Care Rehabilitation Service Pilot Project through Northern Sydney Health.
  • Celebrated 10 years since incorporation of CCNB with a cocktail party following the AGM.

2005

  • Successful tender to Department of Health and Ageing for 25 new community aged care packages, bringing the total number of packages provided by CCNB to 95.
  • Successfully tendered to Department of Health & Ageing for refunding of Dementia Respite
  • Project to provide respite to 16 carers.
  • Participated in DADHC Metro North HACC Advisory Committee re planning.
  • Participated in several DADHC service type reviews.
  • Expanded office space by leasing new meeting room area.
  • Participated in Quality Reporting for Community Aged Care Packages and Dementia
  • Respite Project under the Department of Health and Ageing’s Quality Framework.
  • Engaged consultant, Julie Maloney, to reorganise, catalogue and maintain CCNB Resource Library.
  • Engaged People Advantage (Realise Performance) to develop Role Competency
  • Frameworks, a Performance Management System and revised Job Descriptions for all staff.

2006

  • Adopted new remuneration packages for all staff to aid in attracting and maintaining a skilled and experienced workforce.
  • Commenced negotiations to lease an extra office in Pittwater Place to accommodate projected staff increases.
  • Received funding from Department of Ageing Disability and Home Care for the following projects:
  • New Community Advisory Service for Manly Warringah and Pittwater.
    • Expansion of Community Options Project from 125 to 155 clients.
    • Expansion of Respite Brokerage from 32 to 62 clients.
    • Expansion of Dementia Advisory Service to provide full time position.
    • Received funding from Department of Health and Ageing to provide Respite
    • Project for Employed Carers in Manly Warringah Pittwater.
    • Received funding from Department of Health and Ageing to conduct a research project in Northern Sydney aimed at streamlining referral pathways and identifying initial service system entry points for people with dementia.
    • Completed Northern Sydney Region Dementia Services Strategic Plan and Action Plan.
  • Northern Sydney Dementia Planning and Service Development Project was completed in June.

2007

  • Conducted internal evaluations on projects funded by Department of Health and Ageing (DH&A) and Department of Ageing, Disability and Home Care (DADHC)
  • Registered Five Year Employee Collective Agreement with Department of Workplace Relations
  • Signed lease for expansion of CCNB to Unit 14
  • Participated in the initial trials of the Australian Community Care Needs Assessment (ACCNA) and Carer Eligibility and Needs Assessment (CENA) tools
  • CCNB staff participated in a number of Clinical Redesign working parties with Northern Sydney Central Coast Health
  • Received Cultural Grant from Warringah Council to conduct an Ageing Through Culture project
  • Received Community Service Grants from both Warringah and Pittwater Councils to conduct a Carers' Photovoice Project
  • Contracted by NSW Health to take responsibility for ComPacks data collection in NSW
  • Commenced work on case management research proposal with Dr Michael Fine at the Macquarie University Centre for Research on Social Inclusion
  • Submitted successful tender for Support Coordination for Older Parent Carers Project in Northern Sydney funded through DADHC
  • Submitted successful tender for 5 additional CACPs to commence from January 2007
  • Submitted tender in 2007 ACAR Round for Extended Aged Care in the Home, Extended Aged Care in the Home (Dementia) and CACPs. Results to be announced in September 2007
  • Completed a second organisational culture survey for all staff
  • Received grant from DH&A to conduct research into Streamlined Referral and Early Intervention for people with Dementia
  • Received grant from DADHC to conduct research into service models for people with Younger Onset Dementia with a specific focus on ATSI and CALD populations in Metropolitan North Region.

2008

  • CCNB received funding from DADHC for research into people with Younger Onset Dementia in the CALD and ATSI communities in Northern Sydney.  This research was contracted to Alt Beatty Consultants.
  • DADHC Project Officer conducted Integrated Monitoring Framework for all DADHC funded projects in August 2007 with a very successful outcome.  CCNB awaits the final Action Plan from DADHC.
  • CCNB Quality Management System manual completed and available electronically to staff.
  • Worked with University of WA on a PhD project to develop self-reported quality of life measures for people with dementia.
  • Commenced negotiations with KinCare for the development of an innovative collaboration for direct service delivery.
  • Signed a lease on Suite 10/11 at Pittwater Place to accommodate expansion of CCNB.
  • Purchased a new fleet of 10 motor vehicles.
  • Undertook an organisational restructure including the creation of a third service delivery team.
  • Conducted a series of biannual client surveys as part of CCNB’s Quality Management System.
  • CCNB case management staff participated in Cultural Competency training.
  • Community Care Census with Department of Health and Ageing was completed in April 2008.
  • Case management staff completed TAFE Diploma of Case Management in June 2008.
  • Completed the third annual Organisational Culture Survey with all staff.
  • Submitted a successful Expression of Interest to DADHC for one-off funding to upgrade The Care Manager software and associated data entry to make the program more efficient.
  • New three year contract received from National Respite for Carers Program for Dementia Respite Project from 1 July 2008.
  • Retendered for Transpac at end of current three year contract which was due to expire on 30 June 2008.
